Step by step:
1. Draw a box
2. Place picture in box
3. Colour it in
Your finished!
But seriously, that's pretty much it except the box is usually width of 960 pixels and height is whatever you want the height to be (probably around 4000 pixels)
The avatar has to be within 200x200, so I made fine 180x200 and but a border around the picture I wanted for my avi. Simple.
