This game is really, really, really fun.
The basic gist of it is that you're running under a clock. You have infinite lives, but die and you get a 15 second penalty.
You get time bonuses for destroying a certain quota of enemies, or getting a certain quota of points.
Charged shots slow you down, which means fewer enemies destroyed, but you get more points per enemy because they destroy all the bullets too (and multiply your score for each enemy destroyed beyond the first.)
So, in addition to trying to not die and blow everything up while going at an insane speed, you have to decide when it's worth your time to do charged shots (if you think you can kill enough with the shot to get point bonuses) and when you would be better off keeping your speed up.
Z is regular shot (hold it down), hold X and release for charged shots. The farther forward you are, the faster you move.
Extreme is the mode to play; I'm fairly sure this is how the game was meant to be played. (Normal is slow, Hard is faster, Extreme is super-fast.) Play on normal until you understand how it works, then go nuts on Extreme. I'll go ahead and set the score to beat for Extreme: 1,498,930 with 2205 kills. It was an amazing run - got all the way to level 5.
