LoL Ranked Team Sign-Up

Hello FFR community! First I'd like to say thanks for opening this thread and I hope you are either interested in creating a ranked team, or you simply would like to provide feedback for teams being formed. That being said, let's continue.

This thread isn't just for my personal use, but to give other FFR users the opportunity to create a team. All participating teams will be added to this post and be organized by rank. I will also constantly update this post as Team Captains inform me of new changes to their team.

Of course with other similar set-ups, there will be certain requirements, which are be listed below.

well i flamed some troll in ranked and it was like my 3rd offense. (most recent offense was about 7 months ago ) still warranted a perma ban to the tribunal though.
