My least favorite has got to be difficulty settings that are based solely off of quantitative things--how much time the computer takes to think of its next move in chess, or giving the player more or less HP relative to what the enemies can dish out. You're not facing a beginner in those modes--you're playing an expert that handicaps himself but still knows more tricks than you ever will.
