Video Games - Nightmare Fuel Edition

It's that time of year again. What better way to usher in the Halloween season than by talking about the things that scare(d) us in video games. What gave or gives you nightmares? Was it a piece of background music? A character? A level? A type of boss/enemy? Specific or general, share it here.

Also, it doesn't have to be from the horror genre of video games. Nonhorror games also apply.

I'll start by saying that underwater levels tend to creep me out sometimes, especially in games from the N64/Playstation era and beyond. On the ground, all you have to worry about is what is around you and above you. However, underwater, danger can come from any direction. Not to mention that some forms of aquatic life, whether based on reality or fiction, can be downright scary. From that eel in Super Mario 64 to the gringills of Super Mario Galaxy 1 & 2, water levels have always been places I've dreaded to go.
