Most common cause of that is impacted ear wax, which won't come out naturally unlike regular ear wax. Can happen for a variety of reasons, not always Q-tips, can sometimes be a malfunction in how your body attempts to get rid of excess wax, or size of ear canals (like me, I have unusually small ear canals). The most realistic answer is, see a doctor that can help with impacted wax. Whether it's pressurized water removal (more of a mainstream procedure) or vacuum removal (specialist). If you've found that hot showers have helped for you in the past, consider trying to get it out yourself first. Rubber ball ear syringes can not exert enough pressure to damage something like your ear drum, but has enough to loosen impacted wax for it to come out (slightly weaker form of pressurized water removal). Unfortunately for where I live, there isn't a specialist that can assist me with that removal on my end. I'd have to take a 2-3 hour bus ride to reach a specialist, and I've been mistreated in the past from both kinds of procedures (water removal got me waterlogged ears for a week, specialist accidentally removed scar tissue within my ear. Bled for a whole day, couldn't hear for a week) so I don't really feel comfortable myself visiting one unless it's a problem I can't solve with continued use for like a week. However, I will say my mistreatment's have been a rare case because I've had to get it done regularly since I was 8 years old, but if one is not so easily accessible like in my situation, or think it might help, then it's worth a try. I really know too much about this specific topic. But yeah, having your head in a specific position for most if not all of a night can cause wax to pile in certain spots. Sometimes those spots are the places that prevent you from hearing, which is what it sounds like to me. |

I generally start out on my side then shift through the night and end up pretty much like OP picture
Also can vouch for the ear syringe. If it's really bad you can go to the doctor and they'll use the big scary one on you. I didn't have to travel, they had the equipment at my normal doctors office. One time mine were so bad I actually couldn't hear anything. I usually sleep on my back initially, but I would change to my sides (both left or right) depending on how comfortable I feel. Although I would change between the back and side throughout.

If I'm tired enough to get knocked out instantly, then I'll sleep on my back. Else, usually on my stomach with both arms folded beneath me.
Almost always end up on my side somewhere throughout the night, though. |

Usually on my right side, sometimes on my left, but occasionally on my back if falling asleep on my side isn't working out, which happens sometimes.
