The SM bar doesn't need time to regenerate; it's just a visual effect, the true value of the lifebar is stored and the lifebar as it appears on screen gravitates towards it.
However, the life bar loss from boos and misses is approximately ten times the gain for keeping combo, and in addition there are timing windows for hitting early and late.

Hehe i don't think its bad enough to get a --, it's at least on sync with gap/bpm and stuff and i know its just 16ths with a few 12ths thrown in. The patterns were developed with the technique of "this looks kinda cool" though.
