Simfile DB Submission Error

I've tried many times over the past month or so to submit a song to the Simfile DB. Every time I attempt to upload it, it brings me to the page that reads "File uploaded" or something along those lines, but it never actually shows up in the database.

Has anyone had any similar problems, and/or know how to fix this?

Things to note:
-I have uploaded files here before.
-I am running Mac OS X (Could this be taking part in the error?)
