Well, it's not exactly anime, but some people here might know what I mean when I say "At home, or on 7F".

Of course, there is always that one in Gunslinger Girl, where Rico killed the bellhop. I saw it coming from a mile away, and yet it still managed to get to me. Though I guess that's more of a case of "saddest killing" than "saddest death".
