Spread, almost every high level play does it. There are a few exceptions but the vast majority play spread. Also, Most play up. Baha plays down, a few play that way...most play up.

There are tons of ways to play spread. If you have autohotkey, you can do it with any keys you want. I play it with the vbnm keys, on down. I also think the only reason there are so many more up players than down players is because up is the default, so many players get accustomed to it, and don't want to change.
Up, no screenshrink, 2 hands and 4 fingers on the arrow keys.
Left hand: Index and middle, for <- and V
Right hand: Middle and ring, for v and ->
Somtimes I re-cross my hands somewhat to hit the un-friendly streams, but I don't cross my fingers; I just rotate my hands somewhat to get at the harder notes and slip back to before. It's really tough on stream like {Rose} has, but I've 1000+ combo'd it, along with Wish, Tribal, Hyper Pressure, {Blaze}, and I have some really tricky FC's and such.
It's safe to say I probably have a style MUCH different from most of the "Top Top Players."
