So first of credits to Hakulyte for first pointing this out.
It appears certain speed mods are handled.. poorly, the characteristics of this bug tend to be random dropped misses, epic goodrushes on dense sections you're pretty sure you're nailing, and sometimes some lag spikes/other forms of lag.
First Hakulyte told me they switched from 2.1 speedmod to 1.6 to get a score for a tourney, and as he described his symptoms I figured they were eerily similar to issues I'd been having on high NPS songs.
I switched from 2.65 speed to 2.6 and the issue went away. I've also confirmed that 2.5 -> 2.6 fixed this issue as well

I want to fill this data out more, if you happen to have lag like described in my first paragraph, check your speedmod, I bet it ends in an odd number! If it does raise or lower it to the nearest even numbered speedmod and try again. I'd love to get ss's for hard verification, as well as trusting you guys wont flub up scores to convince me I'm wrong when I might be right here, help me contribute to this so we can solve a possibly gamebreaking issue.
I almost quit again due to pa, others surely have, and this might have been the problem all along.

Edit: for some context on the severity on this bug, I went from 250g quadraphinix with the bug, to 24.0.4.0 without the bug, that's a lot!
Edit2: Considering 2.65 was bad but 2.6 was good what about say 2.652 or another ending number, this may decuce whether its to do with closer to whole numbers, or even vs odd?
