Left hand middle finger-left arrow key
left hand index finger-down arrow key
right hand index finger-up arrow key
right hand middle finger right arrow key
Edit: You could also use the above for left hand but instead of using the arrow keys for the right hand, use the 8 and 6 on your numpad, which would spread them out.Best Scores: Love-Colored Master Spark AAA, Mute City Remix AAA, Over the Rave AAA, TGWP3 3 good.

You... can't? You can use 8 and 6 on the numpad instead of up and right, but that's it.
