Just improvements for the most part. Pleasant farmer i rarely AA so that was nice, Scarhand MA/PA is o_0 for me(still need fewer cbs :( ). The other files I'm just flat out bad at.

Pretty nice Fury of the Storm Chauster, shame about the combo.

I love this file, greats are from me being dumb in the start (I got both greats before I got a perfect :-(). I could most likely improve this MA to mid teens if I'm lucky.

What do you usually get? For me, the end is 50% reading transitions / 50% luck (in terms of not messing up and racing stuff), so it can be tough to get a good score...

1-3- Examples of mentally ill patients who died in private homes for the mentally ill (Randolph Maddix and Lisa Valante- suicide; Gregory Ridges- murdered)
4- NY closed wards and forced ill onto the streets/homes
5- A NY Times extensive report show homes developed into poor places to live
6- Report shows the state has not been taking adequate measures to help the ill
7- Investigation procued the first full accounting of deaths- many died, often premature
8- Two largest homes- Leben Home/Seaport Manor- quarter of residents who died<50 years old
9/1 (page 2)- Many died due to suicides/lack of care
Overall main idea- The homes offer poor living conditions to the mentally ill, and the state has neglected this. The NY Times did a report on the conditions.
2- Mentally ill naturally have shorter life expectancies
3-4- Dr. E. Fuller Torrey, famous psychiatrist, finds the analysis disturbing (many premature deaths for example)
5- Many dead, # of prevented deaths is impossible to know
6-12- NY state department of health- lack of death reports; officials declined to comment, though a debuty health commissioner, Dr. Antonia C. Novello said more investigation would occur (never happened)
Overall main idea- Many deaths could be prevented and the state does not investigate into the deaths enough
1- 15,000 mentally ill adults live in>100 homes in the state
2-4- 60's- wards were closing, homes were created and envisioned to be the solution
5-9- numerous reports were done, the homes were total failures and "flophouses"
10-13- Homes are poorly run and staffed, many lack any knowledge of mental-health training
Overall main idea- Homes are responsible for a large number of people, but they are not adequate for support.
1- Nathaniel Miller- former porter at Surf Manor, claims two horrifying accounts of the ill, but describes them as common
2-3- The ill live in poor conditions, and their (mental) conditions are terrible; many do not survive
4-6- Deaths are common- 1 death every 3-8 weeks, 14/66 dead<50 years old. Seaport has a high rate of death for middle-aged people; one resident commited suicide and Seaport did not disclose true information on the death
7- NY's homes have been terrible for decades- "national tragedy"
8- Though some homes offer good care, many still don't.
9- Homes are costly; government spends ~$40,000/resident per year; adds up to $600 million total/year
10- Government does not hold the homes accountable- "where will the mentally ill live?"
11- Thousands of the ill have lived in poor and chaotic conditions for decades
Overall main idea- The homes have poor living conditions and often result in numerous, preventable deaths
1- Summer of '99 was very hot; soaring temperatures creating poor conditions
2- Leben Home- rooms heat up quickly; hotter indoors than outdoors
3- >350 residents vulnerable to heat and couldn't pay for air conditioning
4- Operator Jacob Rubin recieves >$3 million/year from the government, keeps most of it for his own uses instead of helping the ill
5-6- Tough conditions; rooms were suffocating and residents kept getting sick
7-8- Resident Michael Bonner suffers from the heat and died in a terrible condition
9- State Office of Mental Health provided sufficient care, but they do not see to the homes who have >3x as many people
10-11 Department of Health deputy health commissioner says to keep residents comfortable in weather >85 degrees, but the state does not require it.
Overall main idea- Summer of '99 caused many problems because of heat; cooling is not sufficient. The homes do not provide enough care.
1- Mr. Rubin would not comment; eventually lost his license.
2- Melvin Ryan/Lisa Valante - schizophrenics with a terrible mental state
3- Both tried to commit suicide; eventally did
4- Both jumped out 7th story windows in similar time periods
5- Homes were thought to be a good substitute.
6-11- examples of thsoe who died; suicide.
12- Suicides were left uninvestigated; in other places the inquires would be routinely prompted
Overall main idea- Numerous undocument suicides occured.
1- Valante had been severely sick since her 20's
2- Mr. Ryan lived with his parents but eventually they could not care for him; he has schizophrenia and abuses drugs
3- '97-'98 arrival in Rockland Psychiatric Center, both had already gone through many problems
4- Ryan put under suicide watch at one point
5- Valante wrote graffiti about death/evil
6-7 Ryan/Valante sent to Anna Erika because NY does not want to provide long-term psychiatric care
8- Like many, Anna Erika was cited for poor care
9- Therapists did what they could, but there weren't enough
10- Ryan/Valante suffered psychotic episodes
11-12- Summer of '00, Ryan refused medication, asked to move from 2nd-7th floor and eventually jumped out of the window and died
13-1 (next page)- Valante repeated, jumping out another 7th story window
Overall main idea- Under poor treatment, two residents jumped out of 7th story windows to their deaths
2-4- Vincent Sirangelo, Anne Erika's operator, said the home did everything necessary and they had done nothing wrong.
5-6- Anna Erika administrators gave only single phone calls to the families of Ryan/Valante to inform of their deaths
7-8 Fall of '00, inspectors were to arrive at Seaport Manor and the staff was "preparing" for it, falsifying information.
9- A worker, Toshua Courthan says the administration told the staff to "be creative" to keep their jobs (falsifying info)
10- Courthan eventually fired after reporting the falsified records; suing Seaport
11- The inspectors that day never detected the false files; Seaport remained opened.
12-1 (next page)- The home origianlly created hope for the ill; for them to "become normal"
Overall main idea- Seaport falsified information to keep themselves in the business; the residents live in terrible, miserable conditions.
2- Seaport dubbed as "The new Warehouse for the Insane" in a 1997 study
3- Life inside Seaport was shown after >10 visits, >500 pages of reports, and >50 interviews w/workers/residents
4- During a typical visit, residents can be seen in poor conditions
5- Two residents openly dealt crack; un-safe environment
6- Resident prostitute says it's tough living and she does it to survive
7- Ambulances visit regularly; 93 runs to the home in a 3-month period once
8- Poor staff often abuses the residents; takes advantage
9- One Seaport died about 1/month from '95-'01; at least 79 died
10- A resident, Jerry, said Seaport isn't helping
11- Seaport was "noncompliance in all major areas of operation"
12- Reports cited inadequate staffing/dangerous lapses in medication distribution; investigators refused to examine rooms because they were so fetid; workers could walk past residents without noticing them
Overall main idea- Seaport Manor was a terrible place to live in- there was plenty of filth, neglect, and lack of care for the residents
1- Government officials say they will crack down on Seaport
2-3- Erratic response- Seaport may be left open by installing new operators; otherwise a $20,000 fine
4- Residents remain in neglect and poor conditions
5- Operators- Baruch Mappa, Martin Rosenberg, and Emil Klein says they would not be interviewed
6- Seaport, like many homes, say that the state does not provide enough money
7- Department of Health did not take action of Seaport; helping trouble homes improve conditions was preferred
8- Closure of a home is the last resort, says Robert R. Hinckley, a deputy state health commissioner.
9- The residents of Seaport see life as what they can do to endure
10- In the early 70's, Kingsboro Psychiatric center began to be deinstitutionalized
Overall main idea- Seaport provided insufficient care, though they were not really penalized.
1-2- Mr. Happa, a real estate developer, thought of Seaport while Kingsboro was relocating its patients. The building opened 3 miles away.
3- The idea was shared by Mappa and the state to provide care for the ill
4-5- Mappa and two business partners never had mental health expertise- as a result, trouble arised.
6- Late 70's- Seaport had terrible conditions; no chargers were filed
7- Clinic workers arrived at Seaport, but the number of workers have decreased (20 to 8)
8-9- Past decade- the home has been run by Esther Elizabeth Rosenberg, daughter of one of the operators. She had little experience. Considered incompetent by the state.
10- Nothing has been done with Seaport- state recently levied a $7,000 fine on the home.
11- While Rosenberg was removed, it was allowed for her son-in-law, Seth Fried, as administrator

3- Courthan was fired; sued the home for saying racially insensitive comments
4- 15 current/former workers at Seaport support her account
5-7- Courthan and her sister, Ms. Johnson, began working and were quickly promoted, despite knowing much at all. They were eventually swamped with work.
8- State inspectors knew of the poor evaluations
9-11- One night, the two had to forge files, if necessary, and after a long night of work, files showing a non-existant doctor and false diagonosis were done.
12- Inspectors did not detect faked documents en masse, but they deceted "altered or missing" files.

9- Residents are free to come and go from the home, but the home is required to keep track. If one is missing >24 hours, a report must be filed.
Overall main idea- People often died at Seaport, but many were left unnoticed and unexplained.
1- At the Leben Home, a group of mentally-ill people were led to a van.
2- None resisted; eye surgery was to be performed on them
3- This was repeated throughout '99-'00; 50 operations on >30 residents
4- "eye surgery was like a catchable disease" says Peter Peterson, one of the more alert residents
5- Few complained about eyesight or had noted problems
6- Procedures varied from cataract oeprations to laser surgery- why this was done is still unknown

3- The homes were intended to give the ill a better chance at life, but this had not occured
4- Many the homes have been shown as a place where the ill are taken advantage of
5- There are the operators, who are businessmen, providers, who often have tarnished records, and a nonprofit group who takes advantage of the government payments
6- There are also the ill, who are taken advantage of
7- Velma McFarlane, a former Leben worker, says she had to force the residents into seeing doctors.
8- Residents require an array of services
9- Most of the workers/residents spoke of coercive tactics for selfish wants.
10- The government pretty much led to all of these problems
11- The state were to pay the homes a daily sum to pay for each person's food, shelther, and supervision- a small $28/person

10- Leben workers still recall the eye surgery.
Overall main idea- The government has not sufficed in caring and tending to the residents.
1- Robert Fazio says that they simply just put a laser beam in his eye
2- Dr. Debbi, who operated on Fazio, operated on >30 Leben residents for an 18-month period from '99-'00.
3- Gail Barnabas said Debbi said it was cataracts; he and many others filled the van.

8- A cataract is a cloudy/opaque area in the lens; surgery procedure, etc.
9- Debbi performed about 70 other procedures on residents from other homes; Ocean House, Wavecrest, and Parkview
10- Debbi refused to comment, stating privacy reasons.
11- Jeffrey Edelman, whose family runs Parkview/Wavecrest, prased Debbi saying that he is trustworthy.
Overall main idea- Probably unnecessary operations took place.
1- A complaint was filed with the Health Department after The Times uncovered the eye procedures
2- Most residents who had the residents continue to live at the home, which is now known as Queens Adult Care.

7- Eye procedures were just a fraction of the many costs billed to Medicaid/Medicare for Mr. Dowling; over $150,000 was billed, despite Downling not having any serious physical ailments.

9- A resident at Brooklyn manor received $45,626 in retirement benefits, but Benito Fernandez, the home's operator, took every penny of it.
10- Not the only case- him and associates were noted for mishandling/approprating money.
11- The state refused to renew Hernandez's license.
12-1 (next page)- The case eventually was withdrawn becasue his wife was State Senator SNellie Santiago of Brooklyn
Overall main idea- eye operations did not help some, and costs were very high. In addition, Benito Fernandez took advantage of residents but wound up not getting punished.
2- 2 nonprofit groups sued the department but lost the case.
3- Ann Pegg Biddle says she always had a sense calls were being made to higher-ups and there was political pressure.
4- Fernandez still rusn the home; Senator Santiago was an administrator but eventually she was removed due to falsified records to make her appear like she attended classes for administrators.
5- Brooklyn Manor was found by the city inspection office to be in dismay and was cited for numerous serious violations
6- Fernandez/Brooklyn Manor aren't unique; there are several other operators like him
7- Operators do not need mental health training; the state does not often check backgrounds.
8- In addition to Fernandez, the ill are entrusted to people like Beryl Zyskind.
9- Zyskind ran Ocean house, and stole $120,000. He was eventually sentenced to 30 months in federal prison.
10- Zyskind's brother-in-law, Sherman Taub, took over his place. He was a resigned lawyer after accused of billing law clients of $232,000 for his personal expenses
11- State Commission on Quality of Care for the Mentally Disturbed issued a report on Ocean House stating numerous violations.
12- A company co-owned by Taub's son, Jay, received $420,000 in medicaid payments by submitting improper climas
13- The Manhattan district attorney is also investigating the allegations.
Overall main idea- the homes can be entrusted to poor operators, who can often take advantage of their residents
1- Ocean House is one of the few nonprofit homse, but it has been suggested as being run as a profit-making home
2- The home obtained Lexus luxury sedans, being at least $40,000 each. Residents also slept in filthy rooms at the same time.
3- Taub says every allegation is false
4- Mel P. Barkan, Ocean House's lawyer, says the home paid for Taub's personal expenses, but it was money that was owed to Taub.
5- Barkan says Taub knows what he is doing and is doing a fine job, at that.
6- Taub still runs the home.
7- Business of treating residents has attracted many
8- Ocean house reaped at least $185,000 annually from five providers
9- One of the hospitals paid for space that it never used; another paid for non-existant things, as well.
10- Ocean House stated the hospital (St. John's) simply not used the space it was entitled to.

1- St. John's had billed Medicaid for $300,000 for clinic sessions for Ocean house residents that were social/recreational, not psyciatric
2- One such was an art group; it was 20 minutes and cost $141 per person per session.
3- Two new Hope officials were charged by the state attorney general with defrauding Medicaid of $9 million
4- Nonprofits also keep profiting from the homes.
5- New Horizon Counseling Center pays its executive VP $315,000; they take robust revenue from Medicaid.
6- New York Psychotherapy and Counseling Center had $8.5 million almost entirely from Medicaid.
7- Revenue derives from the relationships with the homes' operators
8-9- At many homes, residents are lined up at least once a month to see a podiatrist.
10- Eileen Marcus claims all they do is clip nails
11- The cost is not insignificant compared to the amount of money spent to feed/supervise residents. $750-$1,000 was used annually for each resident just for podiatric care.
Overall main idea- High amounts of funds are often being spent for "useless" things.
1- 137 names were posted in a morning schedule to see a podiatrist named Dr. David B. Fuchs, costing the government $100 per person.
2-3- Ernest Nelson has been treated for many conditions- Medicaid paid >$2,500 in total.
4- Dr. Fuchs says the patients "have corns, calluses, and pain"
5- Nelson just says money is being made off of them.
6- At Leben, residents are treated by a podiatrist named Dr. Stephen Smirlock.
7- Some administrators think paying podiatrists $50-$75 per visit was appropriate.
8- Benay Phillips claims that cutting somebody else's tonails can be injurious
9- Medical fraud investigators have long been suspicious of the podiatric professions; many have been punished for cheating Medicaid. Podiatrists in homes can still find ways to get around rules.
10- Records still show that the state has just stood by.
11- At Seaport, prescription podiatric creams were discovered in residents' rooms, which is a violation of rules. The reason behind this, however, was never looked into.
12-13 The podiatrists had prescribed so much cream that there was a problem- there wasn't enough room to store psychotropic drugs.
