Play spread(Use four fingers), up your speed mod, keep practicing, etc...

I like it when my hands are in pain, but sometimes, I usually only play a lot in the morning lol , the song that would hurt my hands would be like Death Piano and Pants, but they hurt in a good way =)

TWO THOUZAND COMBO
|
I've AAA'd all but seven of the 11's, Laggy has all but 2 and Rubix has all of them.
so ya some people can do them. takes lots of practice.

Most people who play regularly can pass any song in the game. The real challenge is to Full Combo them or AAA them (Hit every note perfectly). Currently the best I can do is FC Very Challenging (9) songs.

11's are alot easier than 12 imo.
And I mean there's a big interval in difficulty. |

The difficulty jump in each interval (1 > 2.... 9 > 10, 10 > 11, 11 > 12) is bigger with each new difficulty.
Though IMO FMO's are stereotyped. A VC AAA could be "nothing" yet a FMO could be "HOLY CRAP AAA WTF YOUR GOD". It's just one up from VC. Though to answer the title, there are many people who can do 11s.

FMOs are similar to VCs in a way. FMOs just have trickier patterns and most of them go at a fast speed (except for Club and AIM Anthem, they're slow, but they have a lot of jacks in them).
FGO is the ultimate in "HOLY CRAP AAA WTF YOUR GOD". You don't see FGO AAAs every day. I can't PA them, so at a very hard part I just mash and press random buttons.

The way FFR is going, 13-15 footers will never exist. This isn't Stepmania. Even vRofl was a joke.
