Here's my setup:

-Feet propped against a wooden beam under neath the desk
-Forearms resting on the chair's arms (Helps me keep control when vibrating to hit jacks)
-Lean slightly forward to put momentum and weight on my fingers (Also helps me for jacks)
-Arms level with the keyboard
-Monitor about 6 inches above and away from the keyboard
