Sudden Fails

When I play with FlashPlayer 21 SA framerates are okay, however frustratingly sometimes when I play at the middle of the file, without any significant combo breaks or auto fail settings it will fail very suddenly.

It seems like it will fail after one note that was actually hit.

Does this happen to anyone? If there is a thread like this already mods please feel free to delete or merge.

are you accidentally hitting the quit button because if so dont do that
