I just use my middle finger to hit both keys at the same time. Trust me, using two fingers to hit [23] jumps will **** up your ability to play songs above the 6-7 range or so. I should know - I did that for almost a year and a half starting from when I first began playing in earnest, and when I switched to hitting them with just one finger it was like getting a huge skill boost, especially in jumpstreams with lots of [23] jumps.
Hitting up-down trills is tough on raised keys - try using two fingers to hit them (index for down/middle for up). If you have a flat keyboard then just slide your middle finger up and down - it's what I do and it works on all but the fastest trills.
