It's when arrows were 4 frames apart, or less than that (so, 3,2,1, and 0 frames apart).
If you hit them at a certain timing (forgot whether it was late, or normal timing), you'd get an average and then a miss. So you had to hit EARLY in order to get perfects on both arrows..
It's fixed, but it's not fixed in the replay engine.

If you hit 4-framers late (the last perfect frame iirc), you would get an average on the second arrow and miss the first. You could still hit them perfectly on time and still get perfects, however. 3-framers you had to hit early (perfect is still possible), otherwise you would avmiss. However, 2-framers forced you to get a good on the first arrow, otherwise the avmiss would happen. 1-framer, first had to be avg (I don't even think there are any songs with 1-framers anyway). And I think there was a 0-framer in chrono trigger iirc (when it was still on ffr). Or maybe it was a 1-framer...
I think that's basically what the avmisses were. xD
tl;dr:
4-framers you had to hit perfect or early to get a perfect.
3-framers you had to hit early to get a perfect.
2-framers you had to get a good on the first arrow, hitting early.
1-framers you had to get an average on the first arrow.
0-framers is 2 arrows overlapped I believe.
