I don't know if you've played Gitaroo man or not, but if you have, then you might know what I'm doing. It's kind of like that..
There are two hold ideas I thought of, maybe both could be used.
The first consists of your mouse following a spiraling path around the speaker.

The pink is the cursor, where as the spiral is the "hold" note, respectively. As the spiral grows closer to the edge of the speaker, the person must move their mouse in a circular path to keep it on the hold.
Another idea is that the would actually have to click on little "pulses" that come up the spiral to the beat of the music. This can be combined with the above idea.
Another idea is that a little hold "tail" comes up with the pulse that appears out of the speaker. It could be two colors, the first one is the obvious location where your cursor needs to be, and you would have to click AND hold for the length of both the pulse and its tail.

The muffin-looking thing is the hold note that needs to be clicked on. Obviously the color following the pulse is the length that the note needs to be held down.
Now, the final idea is a combination of both the spiral and the click and hold concept. So now, once the pulse reaches its location, the player must click and hold, along with following the path of its hold tail.

I'm pretty sure that only four locations are only allowed due to stepmania limitations, but there should DEFINITELY be a way to put in two or four more. Solo mode and Doubles, anyone? If that was the case, the pulses will have to go to one of those four corners instead of wherever the spiral goes.
